Community Living Support Services

Community Living Support Services help individuals live their day-to-day lives with dignity and independence in their own homes and communities. Hi-Line Home Programs provides one-on-one support that is practical, respectful, and tailored to individual needs.

What We Do

Hi-Line Home Programs provides community living supports for children and adults eligible for Montana’s Medicaid Developmental Disabilities Program (DDP) 0208 Waiver.

Services are designed to help individuals live safely, build independence, and participate fully in their communities. Supports are person-centered and based on each individual’s needs, goals, and approved service plan.

Services are provided where people live, work, and spend their time.

How Support Looks

Our Direct Support Professionals work alongside individuals to support everyday activities, which may include:

  • Personal care and daily routines

  • Skill-building and independence

  • Community involvement and social connection

  • Support with transitions, schedules, and routines

  • Assistance in the home or community as outlined in the service plan

Our Role

Hi-Line Home Programs hires, trains, and supports Direct Support Professionals who provide services under the 0208 Waiver.

We work closely with individuals, families, guardians, and case managers to ensure services are delivered consistently and in alignment with DDP requirements.

If a direct support position becomes unfilled, HLHP staff step in to maintain continuity of services while recruitment is underway. Our focus is on matching the right staff with the right individuals and providing ongoing oversight to ensure services remain reliable, respectful, and person-centered.

To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
